are .env not rendered to the browser then or something? What is to stop someone from viewing the app in their browser and fetching your API key out of the rendered JS with inspect element?

hm the thing I am wondering is, when you pull environment variables (e.g for http requests) out of the environment file, will those values not be seen as cleartext in the code, e.g when you send a network request somewhere?
What if index.js and .env are in different folders?
